Fresh/Re-issue/ Renewal of Passport after normal expiry or pages of the existing valid passport are full
  • EA(P)1 Form - (Download Form)

  • 6 latest studio photographs which should be of square size of not less than 2 x 2 inches (51 mm x 51mm) of high quality colour with light background and the face covering 80 % of the photo.

  • Original Passport with photocopy of relevant pages (photopage, first and last page, Philippine visa page)

  • Original I-card - ACR/ ICR with photocopy (valid)

  • Proof of change of address in India, if applied for inclusion in fresh passport.

  • If married and Name of Spouse is not yet included in previous passport and applicant applies for such inclusion in the new passport .

  • Original Marriage Certificate with photocopy authenticated by NSO and DFA in the case of marriage solemnized in the Philippines or Ministry of external Affairs, New Delhi in the case of marriage solemnized in India

  • Joint Affidavit (from husband and wife) to be sworn before the Consular officer Download Affidavit

  • Joint Photo (during the marriage)

  • Copy of Affidavit of Legal Capacity (for Filipino wife only)

  • Valid ID of spouse

Note 1: In respect of renewal / re-issue of passport of a minor , either of the parents or guardian has to submit the application along with originals of all the documents of the minor which would be returned back to them after acceptance of application

NOTE 2 : In case more than one year has elapsed since the expiry of the passport applicant will also need to submit an affidavit in the prescribed format stating that you have not acquired Filipino nationality and a letter from DFA confirming the same. (Download Affidavit). This may be sworn before the Consular Officer.

LOST/DAMAGED PASSPORT

  • EA(P)1 Form - (Download Form)

  • 6 latest studio photographs which should be of square size of not less than 2 x 2 inches (51 mm x 51mm) of high quality colour with light background and the face covering 80 % of the photo.

  • Original Passport with photocopy of relevant (photopage, first and last page, Philippine visa page) (FOR DAMAGE PASSPORT).

  • Photocopy of lost passport (Front and Back) and other relevant pages with Philippine visa endorsement

  • Original I-Card - ACR/ ICR with photocopy (valid)

  • Police Report on letter head

  • Sworn Affidavit for LOSS/DAMAGE (from the applicant) available at the counter/website and sworn before the Consular Officer (Download Affidavit)

  • Joint Affidavit from 2 Indian witnesses (stating that they have the knowledge regarding the lost/damage passport and certifying applicant’s identity and Indian nationality)

  • Passport photocopies of 2 Indian witnesses (with latest arrival)

  • Photocopies of ACR/ICR – I-card of 2 Indian witnesses with photocopies

  • Letter from the employer certifying applicant’s identity and Indian nationality.

  • If married and name of spouse is not yet included in previous passport and the applicant applies for such inclusion :  

    1. Original Marriage Certificate with photocopy authenticated by NSO and DFA in the case of marriage solemnized in the Philippines or Ministry of external Affairs, New Delhi in the case of marriage solemnized in India

    2. Joint Affidavit (from husband and wife) to be sworn before the Consular officer. (Download affidavit)

    4. Copy of Affidavit of Legal Capacity (for Filipino wife only)

      Process time 3 months if Embassy of India Manila is not the original PIA: one month if lost passport was issued at Manila (Personal interview by the Consul)

3. PASSPORT FOR THE NEWLY BORN CHILDREN
  • EA(P)1 Form - (Download Form)

  • Form of Registration of birth of the newly born child (Download Form)

  • 6 latest studio photographs which should be of square size of not less than 2 x 2 inches (51 mm x 51mm) of high quality colour with light background and the face covering 80 % of the photo.

  • Original Passports with photocopy of relevant (photopage, first and last page, Philippine visa page) of both parents

  • Original I-Card - ACR/ ICR with photocopy (valid) of both parents

  • Original Birth Certificate authenticated by NSO (main office, printed in security paper) and DFA with photocopy

  • The passport application should contain the thumb impression of the child and should be signed by both the parents(Col No28 regarding declaration by parents).

Important Note

The name of parents MUST be included in both passports, IF NOT, they must apply for such INCLUSION
  • 2 passport size photos

  • Original Marriage Certificate with photocopy authenticated by NSO and DFA in the case of marriage solemnized in the Philippines or Ministry of external Affairs, New Delhi in the case of marriage solemnized in India

  • Joint Affidavit (from husband and wife) to be sworn before the Consular officer. (Download affidavit)

  • Joint Photo (during the marriage)

  • Copy of Affidavit of Legal Capacity (for Filipino wife only)

  • Valid ID of spouse

*The name of the parents should be the same as on the Birth Certificate of the Child.

Processing time : 20 days

Note : Registration of children eligible for Indian nationality

For a child born in the Philippines and Federated States of Micronesia, Palau and Marshal Islands to be registered as an Indian citizen , the application must be made within one year of the child’s birth on the prescribed form . In case the child is above one year, the case will be referred to the Ministry of Home Affairs, Government of India for clearance. Birth will be registered and Indian passport issued on receipt of clearance.

An affidavit to be submitted by the parents stating the reason why the child was not registered within one year of birth and the child has not acquired citizenship of any foreign country. If one of the parents is a foreign national, parents will produce a letter from the Department of Foreign Affairs confirming that the child has not acquired citizenship of that country. Embassy (Consular Section) may be consulted for additional application requirements.
