Note 1: In respect of renewal / re-issue of passport of a minor , either of the parents or guardian has to submit the application along with originals of all the documents of the minor which would be returned back to them after acceptance of application
NOTE 2 : In case more than one year has elapsed since the expiry of the passport applicant will also need to submit an affidavit in the prescribed format stating that you have not acquired Filipino nationality and a letter from DFA confirming the same. (Download Affidavit). This may be sworn before the Consular Officer.

Original Birth Certificate authenticated by NSO (main office, printed in security paper) and DFA with photocopy
The passport application should contain the thumb impression of the child and should be signed by both the parents(Col No28 regarding declaration by parents).
Important Note
The name of parents MUST be included in both passports, IF NOT, they must apply for such INCLUSION

Processing time : 20 days
Note : Registration of children eligible for Indian nationality
For a child born in the Philippines and Federated States of Micronesia, Palau and Marshal Islands to be registered as an Indian citizen , the application must be made within one year of the child’s birth on the prescribed form . In case the child is above one year, the case will be referred to the Ministry of Home Affairs, Government of India for clearance. Birth will be registered and Indian passport issued on receipt of clearance.
An affidavit to be submitted by the parents stating the reason why the child was not registered within one year of birth and the child has not acquired citizenship of any foreign country. If one of the parents is a foreign national, parents will produce a letter from the Department of Foreign Affairs confirming that the child has not acquired citizenship of that country. Embassy (Consular Section) may be consulted for additional application requirements.
